Student Request Form
We understand that extenuating circumstances can sometimes interfere with a student’s ability to complete their coursework. In these circumstances, students may request a cohort transfer, leave of absence, or self-disenrollment. Eligible circumstances include, but are not limited to:
- Medical Conditions
- Mental Health Conditions
- Family Loss
- Legal Situations
Cohort Transfers:
Cohort transfers allow students to be moved to the next available cohort. To be eligible for a cohort transfer:
- Students must have 70% or higher in all coursework.
- Students are only allowed 1 cohort transfer during their enrollment.
Leave of Absences:
Leave of absences allow students to pause their enrollment for up to 1 year. To be eligible for a leave of absence:
- Students must have 70% or higher in all coursework.
- Students must have documentation to verify their circumstances.
Self-Disenrollments:
If a student decides that they no longer wish to continue in the program, they may request to self-disenroll. This process officially withdraws you from the program.
How To Submit a Request:

- Make sure to submit accurate information. Missing or inaccurate information could result in your request being denied.
- Students will receive email notification if their request is approved, denied, or if additional information is needed.
