Academic Extensions Request
Students in the Substance Use Disorder (SUD) Counseling Program should speak with their course instructor before submitting a request. The Academic Extension Request form below is for any student in the SUD Program needing an extension on any of their academic assignments. Students shall ensure that their request is completely filled out, accurate information, and in the time deadline necessary to grant extensions.
- Students are responsible for meeting all course deadlines and requirements. Course deadlines and requirements are listed in the catalog portion of your Moodle or can be found here. Academic extensions can only be granted while the course is in-progress.
To Request an Academic Extension
- Complete and submit the Academic Extension Request Form
- Make sure to submit accurate information and detailed reason for request of extension
If your request is approved, you can proceed with submitting your assignment by the new deadline. If your request is denied, your course instructor or the SUD Chair will reach out to you.
